OPD Register with Daily Collection (Excel)
The reception desk's visit register in Excel. Write one row per patient visit: token, name, age, new or follow-up, doctor, fee and how it was paid. The workbook gives the day's patient count and cash, UPI and card collection for every day of the month, totals for each doctor and a list of anything still unpaid, so closing the counter at night takes a minute.
Free · no sign-up · opens in Microsoft Excel, Google Sheets, LibreOffice and WPS Office. Files contain no macros.

What's inside
- Settings sheet: clinic name, month, your doctors, visit types with their usual fee, and payment modes
- Visits sheet with 1,500 ready rows: date, token, time, patient ID, name, age, sex, phone, new or follow-up, doctor, visit type, fee, discount, amount paid, payment mode and balance
- Usual fee fills in from the visit type; you can type over it for special cases
- Day-wise sheet: patients, new and follow-up counts, and cash, UPI, card and total collection for each day of the month
- Doctor-wise sheet: visits, new patients, fees charged, collected and pending for each doctor
- Unpaid balances highlighted in red so nothing is forgotten at the counter
How to use it
- On the Settings sheet type your clinic name and the first day of the month (for example 01-11-2026).
- List your doctors, and check the visit types and their usual fees (New consultation, Follow-up, Review, Emergency, Procedure). Change them to match your clinic.
- Every time a patient registers, add a row on the Visits sheet: date, token, name, age, sex and phone, then pick New or Follow-up, the doctor and the visit type.
- The usual fee appears in the Fee column. Type a discount if the doctor allows one, then the amount paid and the payment mode.
- At closing time open the Day-wise sheet: the cash figure is what should be in the drawer. Count it and compare.
- At the month end print the Day-wise and Doctor-wise sheets, then start a fresh copy for the next month.
Tips
- Give every patient a permanent patient ID (for example P-0001) on the first visit and reuse it on follow-ups, so you can find all their visits with Data › Filter.
- If a patient pays later, add a new row on the day of payment with fee 0 and the amount paid, and note "old balance" in Remarks.
- Patient details are private. Keep this file on your own computer or a drive only your clinic can open, put a password on it (File › Info › Protect Workbook › Encrypt with Password) and never post it in group chats. Follow the health-data rules that apply where you work.
- This is an admin and record-keeping template. It holds no medical advice and does not replace your clinic's medical records or software.
Questions and answers
Can I use it for more than one doctor?
Yes. List every doctor on the Settings sheet and pick the doctor on each visit. The Doctor-wise sheet totals visits and fees for each one, which also helps when you share fees with visiting consultants.
Does it keep the patient's medical history?
No. It is a front-desk register of visits and payments only. Clinical notes belong in the patient's case file or your clinic's medical record system.
Why does the fee change when I pick a visit type?
The Fee column looks up the usual fee for that visit type from the Settings sheet. It is a yellow cell, so you can type a different fee over it for any patient.
How do I find a patient's old visits?
Click any heading on the Visits sheet, use Data › Filter, and filter the Patient ID or Phone column.
